Collective Medical Partners with the Hospital Association of Southern California

Collective Medical, delivering the nation’s largest and most effective network for care collaboration, has partnered with the Hospital Association of Southern California (HASC). Through the partnership, HASC membership, which includes 184 member hospitals and 40 health systems, will have access to the Collective network and EDie care collaboration tool to identify and support complex and vulnerable patients.

EDie enables emergency department (ED) physicians with the ability to overcome a huge road-block in care delivery by doing something seemingly very simple, yet difficult: EDie collects data from all EDs visited by a patient, packages that data into actionable insights and then delivers those insights to ED clinicians via real-time notifications the instant they are needed. Additionally, Collective supports care teams across emergent, inpatient, post-acute, behavioral and ambulatory settings, as well as stakeholders in ACOs and health plans.

“EDie is a hospital standard in Washington, Oregon, the Bay Area, and many other states,” says Kimberly Johnson, association services director for HASC. “We are proud to help bring this technology to Southern California and believe that with it, our members can impact reductions in opioid addiction and lower unnecessary hospital admissions. Insights offered to ED physicians through EDie include information such as:

  • Utilization histories and condition summaries
  • Provider contact information
  • Social determinants
  • Advance directives
  • ED-specific care recommendations
  • Security alerts

The Collective network has demonstrated improved outcomes and lowered costs on an impressive scale. In a Brookings Institution review of Medicaid patients who visited emergency rooms in Washington state, the Collective network and EDie technology—allowing actionable, real-time coordination across organizations—was one of the core strategies for lowering the number of ED visits by patients with patterns of high utilization. By partnering with Collective to support these patients, Washington reported $34 million in savings in Medicaid emergency department costs and a decline of 9.9 percent in emergency department visits in its first year of use in 2013.
