Community Care Cooperative (C3), the largest and most successful non-profit, FQHC-governed ACO in the country, is pleased to announce a major expansion with the addition of 10 Federally Qualified Health Centers (FQHCs) across California, Colorado, Massachusetts, Oregon, Rhode Island, and Washington. This strategic growth strengthens C3’s position as one of the fastest-growing Accountable Care Organizations (ACOs) dedicated to supporting FQHCs to improve health outcomes and reduce costs for communities nationwide.
Effective January 1, 2026, these new participating health centers will join C3’s Centers for Medicare & Medicaid Services (CMS) Accountable Care Organization (ACO) programs, including ACO REACH, the Medicare Shared Savings Program (MSSP), and ACO Primary Care (PC) Flex. With this expansion, C3 now collaborates with 47 FQHCs, collectively managing care for more than 46,000 Medicare beneficiaries under value-based agreements.
This growth reflects C3’s commitment to delivering measurable value—better health outcomes, improved quality of care, and sustainable cost savings—while supporting health centers in their mission to provide person-centered, community-based care.
